We are currently seeking a Independent Living Officer (Help@Hand) to be based at Osbaldwick, York on a Temporary contract until 30 September 2019 for 35 hours per week. The main purpose of this role is to provide a high quality person centred support service to customers, promoting independence and choice. The role will include regularly undertaking needs and risks assessments both dynamically and paper based and ensure outcomes are documented and actioned and providing an agreed level of support to customers, signpost and refer to external services as required, liaising with health, care and other professionals and attending case conferences to facilitate their support The successful candidate will be responsible for the effective running of a sheltered housing scheme, liaising closely with Neighbourhood colleagues. Applicants must have a full driving licence valid for the UK and access to a car.
