Our Ty Ffynu service is able to provide homely accommodation to 4 clients aged 18 and over who require 24/7 support with a diagnosis of Functional Mental illness.
The homes aim to provide a stimulating environment for our clients where they can feel safe and flourish. Thus giving them the greatest opportunity to reach their full potential by providing nurturing, structured and supportive care.
Our service uses a holistic and person centred approach with support from local community teams, Service users are encouraged to make choices and work towards independence. The team work in collaboration with the service user, care co-ordinators and the commissioning team to deliver a package of care that is realistic and speciﬁc to the individual’s needs.
Our focus of care is to help service users to gain conﬁdence and build independence in order to improve their quality of life.
To provide the highest standard of person centred support and care to the people who use our service, maintaining dignity and respect, enabling them to lead as independent lives as possible and realise their potential whilst ensuring own actions and behaviour comply with corporate policy and local procedures, statutory regulations and quality standards.
